Projects Under Construction

The Crossings at Hope Mills
In September 2016, Smith/Packett was awarded a Certificate of Public Need for 100 beds in Cumberland County by the State of North Carolina’s Department of Certificate of Public Need. The 100 beds are being relocated from two previous buildings. The new construction of the 56-unit adult care, and 32-unit memory care is approximately 64,000 sq. ft. It is scheduled to be completed in summer 2018. The Community will be operated by Harmony Senior Services and will feature two courtyards, a town center, and many other amenities for the residents. The location will provide easy access from Rockfish Road to nearby amenities. The Crossings at Hope Mills will meet a growing need in the community as recognized in the Certificate of Public Need, by providing senior living services in Cumberland County, North Carolina.


The Crossings at Morgantown
The Crossings at Morgantown is a new 175 unit independent living and assisted living facility with a dedicated memory care wing in the affluent Cheat Lake community of Morgantown, WV. Named The Crossings at Morgantown, this community is on an 8.9 acre site on Point Marion Road near The Pines Country Club. The facility will benefit from a scenic and convenient location that is within four miles of three hospitals. The community will be managed by Harmony Senior Services. Smith/Packett will begin construction on the project in February 2017 and the community will open in mid-2018. The 180,000 square foot community has a total project cost of $34,000,000.


The Village at Columbia
The Village at Columbia, located on the campus with The Crossings at Columbia, an existing 87 unit assisted living and memory care community (99% occupied), will provide independent living, assisted living, and memory care services in Columbia, SC. The new 109 unit senior living facility, made up of 87 independent living units, 11 assisted living units, and 11 memory care units, will be located on a 5 acre site fronting Clemson Road with conjoined access to The Crossings at Columbia. Once completed, the campus will offer a full continuum of senior housing options. The ability to “age-in-place” on a single campus will be a major advantage for The Crossings and The Village at Columbia. The estimated total project cost is $21,000,000.


The Crossings at State College
The Crossings at State College, a new 112 unit senior living facility with 74 units of personal care and 38 secured units of memory care will be situated on a 14.73 acre site fronting Blue Course Drive across the street from The Penn State Golf Courses. Wessex secured the debt and equity financing for this $24,000,000 project, to be called The Crossings at State College. The community will be operated by Harmony Senior Services. The Facility will benefit from being in close proximity to the Mount Nittany Medical Center, a 243 bed hospital located less than four miles away from the site. The area is heavily influenced by Penn State University, the main driver of economic growth for the region with approximately 46,600 undergraduate students and 26,350 employees. Smith/Packett will begin construction in spring 2017 and the community is scheduled to open in summer 2018.


The Crossings at Brentwood
The Crossings at Brentwood will provide independent living, assisted living, and memory care services in the exclusive Brentwood submarket of the Nashville, TN metro area. A new 191 unit senior living facility with 84 units of independent living, 69 units of assisted living and 38 secured units of memory care will be located on a 10.46 acre site fronting Church Street East across the street from The Estates at Brentwood apartment homes. The area is highlighted by high-end residences, a number of medical centers, and a high concentration of professional offices. The Facility will offer spacious living quarters across all levels of care. The estimated total project cost is $40,000,000. This community is expected to open in Summer of 2018.


Dockside Health and Rehab Center
In December 2016 Smith/Packett was awarded a Certificate of Public Need for 30 additional skilled nursing beds at Dockside Health and Rehab Center located in Middlesex County, Virginia, by the State of Virginia’s Department of Certificate of Public Need. Smith/Packett was awarded the beds after participating in a lengthy competitive application process that included public hearings with the State. The successful application required coordination with both local and state government and received overwhelming community support from Middlesex County and beyond. Smith/Packett’s application was approved in part due to the ability of Smith/Packett and Saber Healthcare Group, the facility’s operator, to deliver projects that provide a high level of patient care and also have a track record of being cost-efficient.

The addition of 30 skilled nursing beds, which will be located in a new purpose-built wing scheduled to be completed in 2018, allows the residents of Middlesex County increased access to quality skilled care. The 30-bed addition follows a multi-million dollar renovation of the existing facility that was completed in 2016. Smith/Packett was contacted by the previous owner to purchase Dockside (formerly Mizpah Healthcare Center), one of the first nursing homes in the state of Virginia dedicated to the care of seniors, due to Smith/Packett’s proven track record of performance.


The Crossings at Martinsburg
The Crossings at Martinsburg will meet a growing need in the community by providing senior living services in Martinsburg, West Virginia. The Crossings at Martinsburg will be operated by Harmony Senior Services 71 assisted living beds, and 32 memory care beds. The 84,194 square foot facility will have 2 floors. The estimated total project cost is $18,695,495 or $181,510 per unit.

The Crossings at Martinsburg will have amenities such as theater, pub, library, main dining room, beauty/barber shop, an exercise room, physical therapy room, and courtyards to provide a quality of care for the residents.


The Crossings at Victory Station
The Crossings at Victory Station is a new senior living community in Murfreesboro, Tennessee, which is located in the greater Nashville market area. The 176-unit project will include 93 independent living, 54 assisted living and 29 memory care residences off Fortress Boulevard near the intersection of Hwy 96. Wessex secured the debt and equity financing for this $31,000,000 project, to be called The Crossings at Victory Station. The community will be operated by Harmony Senior Services. With close proximity to retail, churches and area attractions, this new community will offer residents fine dining with an on–site chef, numerous recreational amenities including a movie theater and pub, 24-hour concierge service, and 24/7 personal care. Smith/Packett will begin construction in October 2016 and the community is scheduled to open in early 2018.


The Crossings at Oakbrooke
Smith/Packett recently closed on the construction loan for a new 181 unit Independent Living and Assisted Living Facility with Dedicated Memory Care Wing in the high-growth Hampton Roads community of Chesapeake, VA. Named The Crossings at Oakbrooke, this community is located at the intersection of Clearfield Avenue and Kempsville Road. This scenic and convenient location is a mile from Chesapeake Regional Healthcare Center and is also adjacent to several Medical and Physician’s offices. The community will be Managed by Harmony Senior Services, adding to Harmony’s already strong presence in the Hampton Roads area. Construction will begin in Summer 2016 and the community will open in Late 2017.

Located next to Oakbrooke Business & Technology Center, The Crossings at Oakbrooke feature 86 spacious independent living apartments, 59 comfortable assisted living apartments, and 36 Harmony Square apartments in a dedicated memory care wing. The Crossings at Oakbrooke feature multiple outdoor courtyards and a walking trail designed to enhance the enjoyment of this Chesapeake Bay community, and offer an assortment of living options and floor plans with one-bedroom, two-bedroom, and studio apartments available. The 188,000 square foot community has a Total Project Cost of nearly $30,000,000.


Liberty Ridge Memory Center
LRMC, LLC is building a new state of the art memory care facility in the Liberty Ridge Community. The 56 unit, 64 bed facility will work collaboratively with Runk and Pratt at Liberty Ridge Independent Living and Assisted Living to provide a campus of continuum of care. It will be operated Runk and Pratt. Partnering with Carter Bank the 38,718 square foot building will have a total project cost of $9,204,585.


The Crossings at Spring Hill
The Crossings at Spring Hill Assisted Living will be a part of the gated active adult Spring Hill Community in Lorton, Virginia. The new assisted living facility will include 74 assisted living units along with a 34 unit memory care wing for a total of 108 units. It is 75,948 square foot building with a total project cost of $18,729,288. Harmony Senior Services will manage the facility situated between Silverbrook Road, Mountain Lakespur Drive and Calla Lilly Court in Lorton, Virginia.


The Crossings at West Ashley
Charleston, SC – Smith/Packett Med-Com, LLC is pleased to announce that their newest community will be located in the Charleston South Carolina market of West Ashley. The 184-unit project will include 91 independent living, 64 assisted living and 29 memory care residences off Glenn McConnell Parkway on Essex Farms Drive. Wessex Capital Investments secured the debt and equity financing for this $34,000,000 project, to be called The Crossings at West Ashley. The community will be operated by Harmony Senior Services, a division of Smith/Packett. With close proximity to I-526 and a block from the Bon Secours St. Francis Hospital, this new community will offer fine dining with an on–site chef, numerous recreational amenities including a movie theater, 24 hour concierge service, and 24/7 personal care. Construction will begin in June 2016 and the community is scheduled to open in late 2017.


The Crossings at West Shore
Smith/Packett recently closed on the construction loan for a new 193 unit Independent Living Facility and Personal Care Home with Dedicated Memory Care Wing in the bustling suburb of Hampden Township near Harrisburg, PA. Named the Crossings at West Shore, the community will be Managed by Harmony Senior Services. Construction will begin in April 2016 and the community will open in late Summer 2017.

The Crossings at West Shore will feature 90 spacious independent living apartments, 72 comfortable personal care apartments, and 31 Harmony Square apartments in a dedicated memory care wing. The Crossings at West Shore feature an assortment of living options and offer multiple floor plans with one-bedroom, two-bedroom, and studio apartments available. The 193,000 square foot community has a Total Project Cost of over $30,000,000.


Pheasant Ridge Memory Center
PRMC, LLC is building a new state of the art memory care facility in the Pheasant Ridge Community. The 56 unit, 64 bed facility will work collaboratively with The Village at Pheasant Ridge to provide a campus of continuum of care. It will be operated by Harmony Senior Services. The site has mountain views and connected to the Pheasant Ridge campus by a walking path. Partnering with Carter Bank the 37,168 square foot building will have a total project cost of $8,477,501.


The Crossings at Five Forks
Smith/Packett recently closed on the construction loan for a new 172 unit Independent and Assisted Living Facility with Dedicated Memory Care Wing at the Intersection of Five Forks Road and Woodruff Road in the growing Simpsonville suburb of beautiful Greenville County, SC. Named the Crossings at Five Forks, the community will be Managed by Harmony Senior Services. Construction will begin in July 2015 and the community will open in December 2016.

The Crossings at Five Forks will feature 100 spacious independent living apartments and 48 assisted living apartments. The community will also contain 24 Harmony Square apartments in a dedicated memory care wing. The Crossings at Five Forks feature a variety of living options and offer multiple floor plans with one-bedroom, two-bedroom, and studio apartments available. The 183,000 square foot community has a Total Project Cost of over $25,000,000.


The Crossings on the Peninsula
Smith/Packett recently closed on the construction loan for a new 102 unit Assisted Living Facility with Dedicated Memory Care Wing in the beautiful Tabb district of historic York County, Virginia. Named the Crossings on the Peninsula, the community will be Managed by Harmony Senior Services. Construction will begin in July 2015 and the community will open in Summer 2016.

The Crossings on the Peninsula will feature 68 spacious assisted living apartments and 34 Harmony Square apartments in a dedicated memory care wing. The Crossings on the Peninsula feature a variety of living options and offer multiple floor plans with one-bedroom, two-bedroom, and studio apartments available. The 86,000 square foot community has a Total Project Cost of over $18,000,000.


The Crossings at Wescott Plantation
North Charleston, SC – Smith/Packett Med-Com, LLC is pleased to announce that their newest community will be located in the Charleston South Carolina market of Summerville. The 184-unit project will include 93 independent living, 57 assisted living and 34 memory care residences and will anchor a new development area in Wescott Plantation with a unique “Town Center” concept design. The $28,000,000 project, to be called The Crossings at Wescott Plantation, will be operated by Harmony Senior Services, a division of Smith/Packett. Within walking distance to Harris Teeter, Lowes and The Golf Club at Wescott Plantation, this new community will have fine dining with an on–site chef, numerous recreational amenities including a movie theater and walking trails, 24 hour concierge service, and 24/7 personal care. Construction will begin in June 2015 and the community is scheduled to open in early fall of 2016.


 

Construction Complete

The Crossings at Chantilly Closed: November 8, 2013
The Crossings at Chantilly will meet a growing need in the community by providing senior living services on a full-continuum senior housing campus in Chantilly (Fairfax County), Virginia. The Facility will be located on a senior housing campus which will include a 166 bed nursing home managed by Commonwealth Care of Roanoke. The Crossings at Chantilly will be operated by Harmony Senior Services and will feature 160 independent living units, 36 assisted living beds, and 24 memory care beds. The 163,254 square foot facility will have 4 floors and an underground parking garage. The estimated total project cost is $30,364,336 or $189,777 per unit. The campus will also feature landscaped roads, a grand entrance, wide sidewalks, surrounding trees, and storm retention on site. The Crossings’ location provides easy access to nearby amenities and a private setting created from the surrounding greenery.


The Crossings at Hanover
Smith/Packett recently closed on the construction loan for a new 106 unit Assisted Living Facility with Dedicated Memory Care Wing directly across from Bon Secours Memorial Regional Medical Center in the high-growth Mechanicsville area of Hanover County, VA. Named the Crossings at Hanover, the community will be Managed by Harmony Senior Services. Construction will begin in June 2015 and the community will open in Summer 2016.

The Crossings at Hanover will feature 75 spacious assisted living apartments and 31 Harmony Square apartments in a dedicated memory care wing. The Crossings at Hanover feature a variety of living options and offer multiple floor plans with one-bedroom, two-bedroom, and studio apartments available. The 90,000 square foot community has a Total Project Cost of over $15,000,000.


The Crossings at Reynolds Mountain
Buncombe AL Investors, LLC is the new owner of approximately 7 acres as of February 12, 2015 off of Cobblers Way in Reynolds Mountain. The 90 unit Independent Living, 58 units assisted living and 24 unit memory care facility will be called The Crossings at Reynolds Mountain operated by Harmony Senior Services. The site is located on side of a hill with views of the mountains. Partnering with Synovus Bank the 170,430 square foot building will have a the total project cost of $32,379,408.


The Crossings at Independence
Smith/Packett recently closed on the construction loan for a new 180 unit Independent Living and Assisted Living Facility with dedicated Memory Care Wing in the Princess Anne neighborhood of coastal Virginia Beach, Virginia. Named The Crossings at Independence, the community will be Managed by Harmony Senior Services. Construction will begin in January 2015 and the community will open in Spring 2016. The Crossings at Independence will feature 100 spacious independent living apartments as well as 56 assisted living apartments and 24 Harmony Square apartments in a dedicated memory care wing. The Crossings at Independence features a variety of living options and offers multiple floor plans with one-bedroom, two-bedroom, and studio apartments available. The 180,000 square foot community carries a Total Project Cost of over $28,000,000.


Coral House Assisted Living Closed: July 10, 2014
Coral House Assisted Living is located in the growing retirement area of Fort Myers/Cape Coral, Florida. The site at 3501 Hancock Bridge Parkway is in a premier location with waterway views, nearby retail, dining, and easy access to both Fort Myers and Cape Coral. The facility will be managed by Ohio-based Saber Healthcare Group, which has an excellent record for providing high quality skilled nursing and assisted living. Coral House will feature 119 private assisted living (87) and memory care (32) apartments in over 90,000 square feet. The spacious apartments offer several one-bedroom and studio floor plans designed to meet the needs of each resident. Luxurious common amenities available to the residents include multiple dining venues, large activity spaces, and an exercise room. The facility is projected to open in September of 2015.


The Crossings at Harbour View Closed: June 6, 2014
The Crossings at Harbour View features 174 units of Independent Living and Assisted Living with a dedicated memory care wing in the thriving Harbour View community of Suffolk, VA. The Crossings at Harbour View is managed by Harmony Senior Services and is projected to open in June 2015. The Crossings features 90 independent living apartments, 63 assisted living apartments, and 21 Harmony Square apartments in a dedicated memory care wing. The spacious apartments provide a wide range of one-bedroom, two-bedroom, and studio floor plans designed to meet the needs of each resident. The 173,000 square foot facility has a Total Project Cost of $26,878,413.


Wythe Health and Rehabilitation Center Closed: March 14, 2014
The Wythe Health and Rehabilitation Center features 120 beds of skilled nursing with a dedicated memory care wing in the town of Rural Retreat, VA. Wythe Health and Rehabilitation is managed by Traditions Management and is projected to open in July 2015. The Wythe Health and Rehabilitation Center will be a newly constructed skilled nursing facility with a dedicated rehabilitation neighborhood for short-term residents recovering from medical operations. There are private and semi-private rooms designed to meet the needs of each resident. The 50,609 square foot facility has a Total Project Cost of $11,500,000.


Abingdon Assisted Living Closed: January 21, 2014
Abingdon Assisted Living will meet a growing community need by providing senior living services in Abingdon (Washington County), Virginia. The new assisted living facility will include 57 assisted living beds along with a 24 bed memory care wing for a total of 81 beds in 72 units. The Facility will be located on a senior housing campus and less than one mile from a new 116 bed acute care hospital: Johnston Memorial Hospital. The Facility will be managed by Twenty/20 Management, Inc. The estimated total project cost is $9,600,256 or $133,337 per unit.


The Crossings at Blacksburg Closed: January 16, 2014
The Crossings at Blacksburg offer 91 spacious units of Assisted Living with a dedicated memory care wing located next to Lewis Gale Hospital in beautiful Blacksburg, Virginia. Managed by Harmony Senior Services, The Crossings at Blacksburg is projected to open in December 2014. The Crossings features 64 assisted living apartments and 28 Harmony Square apartments in a dedicated memory care wing. The Crossings at Blacksburg feature a variety of lifestyle options with one-bedroom, two-bedroom, and studio apartments available. The 67,000 square foot facility has a Total Project Cost of $10,750,000.


The Crossings at Columbia Closed: November 25, 2013
The Crossings at Columbia feature 89 units of Assisted Living with a dedicated memory care wing located in the growing Killian community in Northeast Columbia, SC. The Crossings at Columbia is managed by Harmony Senior Services and is projected to open in January 2015. The Crossings features 65 spacious assisted living apartments and 24 Harmony Square apartments in a dedicated memory care wing. The Crossings offers multiple floor plans with one-bedroom, two-bedroom, and studio apartments available based on the needs of each individual resident. The 69,000 square foot facility has a Total Project Cost of $10,400,000.


Dulles Health and Rehab  Center Closed: November 8, 2013
Dulles Health and Rehab Center will be located on a senior campus with The Crossings at Chantilly independent living, assisted living and memory care neighborhoods. The Facility will be managed by Commonwealth Care of Roanoke. It will feature 166 beds with private and semi-private rooms designed to meet the needs of each resident including a dedicated area for rehabilitation services. The 83,720 square foot facility will have 2 floors with a total project cost of $23,827,332.


Liberty Village Closed: December 13, 2012
Smith Packett is excited to announce the completion of construction and opening of a new senior housing building with 173 independent living, assisted living and memory care apartments located in Lynchburg, Virginia adjacent to Liberty University. The new 165,850 square foot independent and assisted living community has 4 stories located at the top of Candler’s Mountain with spectacular views. The building’s amenities include dining rooms, living rooms, exercise facility, chapel, pub, activities rooms, bowling alley, beauty barber spa, billiards, and much more. This new community operated by Runk & Pratt is on a campus which also includes a 90 bed skilled nursing facility developed by Smith Packett in 2013.


Kempsville Health and Rehabilitation Center Closed: November 13, 2012
Smith/Packett recently completed construction on a new 60 bed skilled nursing wing at Kempsville Health and Rehabilitation Center located in coastal Virginia Beach, Virginia. Managed by Saber Healthcare Group, the new wing opened in June 2015. Kempsville Health and Rehabilitation Center features 90 skilled nursing beds with private and semi-private floor plans. The project included a transfer of 30 CON-issued skilled nursing beds that were then combined with 60 existing beds. The 30 bed transfer required approval from the Virginia Health Department, which included a public hearing. The 27,500 square foot addition and renovation carries a Total Project Cost of $6,087,000.

 

Recent Closings

The Crossings at University Park / The Pemberton at University Park Closed: November 6, 2015
Smith/Packett announced the successful sale of the University Park campus near Richmond, Virginia to Kayne Anderson Real Estate Advisors. The sale, which closed on November 6, 2015, included The Crossings at University Park, a 93 unit assisted living and memory care facility, The Pemberton at University Park, a recently completed 124-residence independent living building and surrounding undeveloped property. Smith/Packett’s affiliate, Harmony Senior Services, LLC, will continue to manage both facilities.


The Crossings at Bon Air Closed: September 11, 2015
Smith/Packett successfully completed the sale of The Crossings at Bon Air, a 186-residence independent living, assisted living and memory care community in Richmond, Virginia, on September 11, 2015. The property was purchased by Griffin-American Healthcare REIT III for $64,000,000, a cost of $344,000 per unit. The transaction included the assumption of $37,650,000 in Fannie Mae debt.


Locust Hill Closed: May 1, 2015
Smith/Packett recently closed on the renovation loan for a 64-bed Skilled Nursing Facility located on the beautiful Rappahannock River in Middlesex County, VA. Named Dockside Health & Rehab Center, the community will be Managed by Saber Healthcare Group. The renovation will begin in June 2015 with work scheduled to be completed in early 2016.

Dockside Health and Rehab Center will contain a new spacious physical therapy center as part of the renovation as well as a secure outdoor courtyard with views of the Rappahannock. Dockside will offer a variety of living options with private and semi-private rooms available. The facility will be Medicare Licensed and offer outpatient therapy services.


Mainstreet Closed: May 19, 2014
Smith/Packett is pleased to announce that it successfully completed the sale of four properties to HealthLease Properties Real Estate Investment Trust on May 16, 2014.  The portfolio consisted of two skilled nursing facilities and two assisted living facilities totaling 365 beds, including one property each in Virginia and Pennsylvania and two in North Carolina, with a purchase price of $49.4 million.


Abingdon Health and Rehab Closed: May 9, 2014
Smith/Packett successfully completed the sale of Abingdon Health & Rehab Center, a 120-bed skilled nursing facility in Abingdon, Virginia, on May 9, 2014.  The property was purchased by an affiliate of the facility’s manager for $14,099,842.
